Referring a Client
A collaborative approach is paramount to improving and maintaining client well-being.
As such, I hope that the below information assists a seamless referral process. Please notify the client that they will need to reach out to schedule an appointment.
This is due to privacy reasons, as not all clients wish to activate their referrals as well as wanting to respect client privacy by ensuring appropriate contact details are provided.
Mental Health Care Treatment Plan
The Mental Health Care Treatment Plan (MHTP) allows for a total of 10 Medicare subsidised sessions per calendar year (6 following initial referral + 4 following a re-referral). You will receive a letter following the client's 6th session requesting the re-referral and outlining client progress. For MHCP referrals, please make sure that a referral letter is sent alongside the MHTP form.
The letter must include:
- The client's name, date of birth, address, presenting symptoms and diagnosis;
- How many sessions the client is being referred for (maximum of 6 at a time);
- Your provider number.
As Medicare only subsidises a total of 10 sessions per calendar year, clients often continue therapeutic engagement despite the MHTP expiry.
You will receive a letter confirming the conclusion of 10 sessions as well as a notification when the client is eligible for a new MHTP.
Eating Disorder Treatment and Management Plan (EDP)
The EDP provides a rebate for total of 20 sessions over 12 months (10 following initial referral + 10 after review. A further 20 appointments can be accessed if the client is then reviewed by a psychiatrist).
Referral expires after 12 months, regardless of how many sessions have been used.
As with the MHCP, please make sure that a referral letter is sent.
The letter must include:
- The client's name, date of birth, address, presenting symptoms and diagnosis;
- How many sessions the client is being referred for (maximum of 10 at a time);
- Your provider number and clinic details.
You will receive a letter following the client's 10th session, requesting the review and re-referral and outlining client progress. A further treatment letter will be sent once 20 sessions are completed.
If the client is no longer eligible to be seen under an EDP, they are welcome to continue engagement under the MHCP or privately.
Chronic Condition Management Plan
For clients dealing with chronic conditions, a Chronic Condition Management Plan allows for an additional 5 rebate sessions per calendar year.
The current rebate is $61.80. The 5 sessions can be used across other allied health services, not just psychology.
It is up to the GP's clinical judgement if the client may be eligible for the CCMP.
There is no list of eligible conditions, but the client must present with at least one condition that has been, or is likely to be, present for at least six months.
Non-Medicare Rebated Referrals
Other allied health providers are welcome to refer.
Rebates are not available for non-Medicare referrals.
Your client may be eligible for private health fund rebates, dependent on their PHI.
